New Patient Information

Welcome to Pediatric Endocrine Associates. My name is Leontyne Price and I am the New Patient Coordinator for the practice. To ensure that your appointment is scheduled in a timely manner the following information is required.

Regardless of your insurance type, all patients must be medically referred to this practice. This requires that your Primary Care Physician (PCP) provides us with all the supporting documents for the diagnosis. Once that information is received one of our physicians evaluates the information determines an appointment timeframe.

Please be aware that due to the volume of referrals this office receives it may take up to two weeks after we receive your PCP’s information to review the material. Then depending upon your diagnosis it may be take another week or so to schedule an appointment. You may contact me at any time to check on the status of your appointment. We receive a lot of calls, so the best way to reach me is to leave a message on my extension, 108. I will return your call.
